How to report an unsafe product or an injury

Last reviewed October 31, 2026

Who can report

Anyone can report a harm or risk of harm from a consumer product at SaferProducts.gov, which is run by the U.S. Consumer Product Safety Commission (CPSC). Parents, caregivers, doctors and others can all submit a report.

What to have ready

Have the product name, brand, model number and any date code ready, along with what happened and when. A photo of the product and its label helps. Keep the product itself if you can, in case it is needed.

What happens next

The agency reviews reports, and reports that are accepted are published in a public database. Reports can help the agency spot patterns that lead to recalls.

Food, medicine and vehicle products

Problems with food, medicine or cosmetics go to the FDA. Problems with car seats and other vehicle equipment go to the National Highway Traffic Safety Administration (NHTSA).

If someone is hurt

Get medical help first. For possible poisoning, call Poison Control at 1-800-222-1222.
